In-office visits:
The initial appointment typically includes a thorough medical and health history intake, general
and/or focused physical exam, and vitals (e.g., measurements of blood pressure, weight, height,
and body mass index). Recommendations for labwork may be given if necessary. Dr. Roxas uses
certain specialty laboratories including Diagnos-Techs, ZRT Laboratory, Doctor’s Data and Genova
Diagnostic Lab. The patient may be asked to log his/her current daily diet for the next week.

Initial visit summary:
- Review of patient history and pertinent lab results
- Petinent physical examination
- Assessment of patient’s current situation
- Establishing health goals
- Recommended diagnostic lab tests (if necessary)
- Personal Diet Journal assessing patient's current daily diet over the next 7 days
- General recommendations may be given at this time

A customized treatment plan is designed after all information has been gathered and necessary
research has been completed. This plan will be presented at the patient's first follow-up visit.

The initial treatment plan details the following:
- Diet and lifestyle considerations
- Recommended nutritional/botanical supplementation
- Recommended diagnostic lab tests (if necessary)
- Other additional specific therapeutic considerations, such as homeopathics, hydrotherapy
instructions, specific stress management exercises, and the like may apply.

New Patient In-Office Visit:
Total Time: 60 minutes
Fee: $120

Follow-up/Established Patient In-Office Visits:
Total Time: 20-30 minutes
Fee: $50

Initial Phone Consult:
Time: 50-60 minutes Fee: $120

Follow-up Phone Consult:
Time: < 5 minutes Fee: Free
Time: 5-15 minutes Fee: $25
Time: 15-30 minutes Fee: $50
Time: 30-45 minutes Fee: $75
Time: 45-60 minutes Fee: $100
Time: >60 minutes Fee: An additional $25 for each 10 minute interval is charged after the first hour

Initial Email Consult:
Flat Fee: $75 for intial health assessment and treatment plan

Follow-up Email Consult:
Flat Fee: $30 (includes up to 5 queries within 30 days of last treatment plan)

Note: Phone appointments must be prescheduled and may be paid for prior to the time of
appointment with either cash, check, or money order.
